The Pilates Krewe II LLC is registered with the State of Florida as a Health Studio.
Registration No. is HS15303. Located at 2108 N. Tampa Street, Tampa, FL 33602.

The Pilates Krewe III LLC is registered with the State of Florida as a Health Studio.
Registration No. is HS15941.  Located at 9568 W. Linebaugh Ave, Tampa, FL 33626.

Collectively, these are referred to hereafter as the “Studios.”

This is a contractual agreement between the Studios and:

PLEASE READ THIS AGREEMENT CAREFULLY. IT CONTAINS IMPORTANT INFORMATION REGARDING OUR POLICIES AND YOUR OBLIGATIONS.

Membership Terms

We are a judgment-free studio. We expect members to support one another through positive affirmation, kindness, and inclusion. We do not tolerate bullying, derogatory language, harassment, or judging others based on their body type, appearance, skill level, or otherwise. Please do not join this studio if you cannot agree to look at others with a positive attitude. Members that do not adhere to this policy may have their membership terminated.

  • Billing, Payment Types & Fees
      • Your initial term is one month, or “Cycle.”
      • Membership costs vary depending on location and enrollment in various programs. The cost will be provided based on these variables before enrolling. There may be additional costs, including taxes and fees. You will be billed on the first day of your membership and approximately every month on the same calendar day as your first day. Membership fees are subject to change in the future.
      • Members receive sixteen (16) credits for reserving classes each Cycle. 
      • Each class is one (1) credit.
      • All sessions must be used within one Cycle.
      • Unused credits expire at 11:59 p.m. Eastern Time on the last day of the current Cycle.
      • The initial contract will not be for more than one calendar month or Cycle. Renewal contracts may not be executed, and the fee is therefore paid only when the preceding contract expires.
      • Memberships auto-renew each Cycle.
      • There is no annual prepayment option; all memberships are billed every Cycle.
      • Memberships may be canceled with 30 days written notice.
      • Any notice requirements shall be provided in accordance with paragraph 10 below.
      • A one-time initiation fee is payable with the first period’s membership fees.
      • Payment by ACH (bank account) is available at no additional charge.
      • Credit card payments will incur a 3% fee to help offset the processing fees.
      • “No Show” A $19 fee is charged to members who are no-shows for a class or cancel in arrears.
      • “Late Cancel Fee” A $14 fee is charged to members who cancel within 12 hours of the class date and time or who show up more than eight (8) minutes late to a class. 
      • If you arrive more than eight minutes late to a scheduled class’s start time, you may be denied entry.
      • “Extra Late Cancel Fee” Members who cancel within 1 hour of the class date and time will be charged a $19 fee.
      • If you cancel your class within 12 hours and register for another (and attend that new class), you will still be assessed a $14 or $19 late cancellation fee.
      • Late cancels, late arrivals, and no shows will also result in a loss of that credit.
      • Members may be assessed a fee of $35 for returned checks or failed credit card transactions.
      • Members may pause their membership for up to one Cycle per year. During this time, members will still be required to pay all applicable membership fees. However, your 16 credits will be available spread across four additional cycles upon your return.
      • If you cancel your membership during the first 10 days of your freeze period, you do not need to give additional notice. You will not be billed further, and your credits will expire at the end of the billing period.
      • If you cancel your membership after the first 10 days of your freeze period, you must provide 30 days’ notice. You will only have your standard 16 credits with no other credits during that time.
      • Members can reserve up to four classes in advance. Members may book another class until they complete at least one class.
      • Class sizes are not guaranteed and may be increased or decreased without notice.
      • The number of members may be increased or decreased without notice. The Studio is not obligated to keep the membership at a certain number.
      • Memberships may not be shared.
      • The Studios reserve the right to terminate a membership anytime for any reason. A pro-rated refund will be issued for unused credits of 1/16th of the membership fee per remaining credit, except for credits received for a freeze period.
  • Classes, Reservations, Waivers & Instruction
      • Classes must be reserved and canceled using our website or mobile application (Android or iOS). No other method of class scheduling is accepted.
      • Classes may be canceled or modified without notice for any reason. The Studio will make its best effort to notify members of this event.
      • Instructors may be substituted without notice.
      • Members must be approved to take advanced-level classes at the sole discretion of the Studios.
  • General Information & Policies
      • Members must review and sign a general liability waiver.
      • Members who are under the age of 18 must have their parent or legal guardian review and sign the general liability waiver.
      • Members who are pregnant must review and sign an additional pregnancy waiver. Upon learning of a pregnancy, the member must notify the Studio in writing of this event.
      • All members consent to be recorded for security purposes. The footage is not public.
      • All members consent to appear in photos that may be placed on the Studio’s social media sites, www.pilateskrewe.com, or on the internet.
      • All members must review, sign, and comply with a policy regarding COVID-19 and other infectious diseases. This includes agreeing to the possibility of temperature checks and masks upon arrival and participation in sanitizing their equipment and station after completing a class.
      • Parking is available in the building lot and adjacent streets on a first-come, first-served basis. There is no reserved parking. Review the parking maps on our website.
  • Facilities Policies
    • Outside footwear must be removed promptly upon entry to the Studios.
    • Grippy socks must be worn at all times in the studio.
    • Members are encouraged to come dressed for participation.

State of Florida Required Disclosures

  1. Members are entitled to the penalty-free cancellation of this contract within 3 days, exclusive of holidays and weekends, of its making, upon the mailing or delivery of a written notice to the health studio, and refund upon such notice of all monies paid under the contract, except that the health studio may retain an amount computed by dividing the number of complete days in the contract term or, if appropriate, the number of occasions health studio services are to be rendered into the total contract price and multiplying the result by the number of complete days that have passed since the making of the contract or, if appropriate, by the number of occasions that health studio services have been rendered. A refund shall be issued within 30 days after receipt of the notice of cancellation made within the 3-day provision.

  2. Members are entitled to the cancellation and refund of this contract if the Studio business location (designated above) closes, goes out of business, or the Studio moves its facilities more than five (5) driving miles from the location designated above and fails to provide, within 30 days, a facility of equal quality located within five (5) driving miles of the location designated above at no additional cost to the buyer.

  3. Members should be advised that any notice of his/her intent to cancel shall be given in writing to the health Studio. Such a notice of cancellation shall also terminate automatically the consumer’s obligation to any entity to whom the health Studio has subrogated or assigned the consumer’s contract. If the health Studio wishes to enforce such a contract after receipt of such notice, it may request the department to determine the sufficiency of the notice.

  4. Members should be advised that if the department determines that a refund is due the buyer, the refund shall be an amount computed by dividing the contract price by the number of weeks in the contract term and multiplying the result by the number of weeks remaining in the contract term. The business location of a health Studio, as designated above, shall not be deemed out of business when temporarily closed for repair and renovation of the premises: 
    1. Upon sale, for not more than 14 consecutive days; or 
    2. During ownership, for not more than seven (7) consecutive days and not more than two (2) periods of seven (7) consecutive days in any calendar year.

      A refund will be issued within 30 days after receipt of the notice of cancellation.

  5. Members are advised to contact the Florida Department of Agriculture & Consumer Services for information within 60 days should the health Studio go out of business to address any unresolved conflicts.

  6. This contract may be canceled if the buyer dies or becomes physically unable to avail himself or herself of a substantial portion of those services which he or she used from the commencement of the contract until the time of disability, with a refund of funds paid or accepted in payment of the contract in an amount computed by dividing the contract price by the number of weeks in the contract term and multiplying the result by the number of weeks remaining in the contract term. The buyer or the buyer’s estate seeking relief under this paragraph may, at the Studio’s sole discretion, be required to provide proof of disability or death. A physical disability sufficient to warrant cancellation of the contract by the buyer shall be established if the buyer furnishes to the health studio a certification of such disability by a physician licensed under Chapter 458, Chapter 459, Chapter 460, or Chapter 461 to the extent the diagnosis or treatment is within the physician’s scope of practice. A refund shall be issued within 30 days after receipt of the notice of cancellation made pursuant to this paragraph.

  7. SHOULD YOU (THE BUYER) CHOOSE TO PAY FOR MORE THAN ONE (1) MONTH OF THIS AGREEMENT IN ADVANCE, BE AWARE THAT YOU ARE PAYING FOR FUTURE SERVICES AND MAY BE RISKING LOSS OF YOUR MONEY IN THE EVENT THIS HEALTH STUDIO AND/OR THIS BUSINESS LOCATION CEASES TO OPERATE. THIS HEALTH STUDIO IS NOT REQUIRED BY FLORIDA LAW TO PROVIDE ANY SECURITY, AND THERE MAY NOT BE OTHER PROTECTIONS PROVIDED TO YOU SHOULD YOU CHOOSE TO PAY IN ADVANCE.

  8. The initial contract will not exceed 36 months and thereafter shall only be renewable annually. Renewal contracts may not be executed, and the fee, therefore, paid until 60 days or less before the preceding contract expires. 
  9. If the health Studio requires a buyer to furnish identification upon entry to the facility and as a condition of using the services of the health Studio, the health Studio shall provide the buyer with the means of such identification.
